FRESNO, Calif. —

Relatives of a 24-year-old intern killed by a lion at a California animal park said Friday they believe the facility followed safety protocols and the death was a tragic accident.

Investigators believe the 5-year-old male lion lifted the door of a partially closed feeding cage with its paw and killed Dianna Hanson as she cleaned a bigger enclosure area, Fresno County Coroner David Hadden has said.

Family members said they’re relieved the young woman was killed instantly when the lion swiped or lunged at her.

Mexico City (CNN) — Ten people died Tuesday in a blaze at a Mexican refinery near the Texas border, the state oil company Pemex said.

Investigators were working to determine what caused the accident, which occurred near Reynosa, Mexico, Pemex said.

A fire that occurred after the blast had been extinguished as of Tuesday afternoon, the company said in a Twitter post.

“The fire is extinguished, and the situation is under control,” Pemex said.

WEST PALM BEACH — Authorities have identified the local construction worker who died Thursday night, hours after falling at least 15 feet while working inside the Palm Beach Mall.Juan Arias had just celebrated his 38th birthday Wednesday. Thursday, he toppled 15 to 20 feet from a scissor lift, an aerial platform often used by maintenance and construction workers, onto the concrete floor inside the mall, said Allan Ortman, West Palm Beach city spokesman.Arias worked for Best-Tec Asbestos Abatement, a company based in Boynton Beach, Ortman said.Arias was removing asbestos near the old Sears automotive garage at the mall, which is on Palm Beach Lakes Boulevard between Interstate 95 and Congress Avenue. Fire Rescue crews were dispatched just before 4:30 p.m Thursday.

STANLEY, N.C. (AP) – Authorities in Gaston County say a 35-year-old man was killed when a tree he was cutting down landed on top of him after it fell into a second tree.

Gaston County Police Sgt. Myron Shelor said Michael Scott Wyatt was in Stanley cutting down a large tree on an embankment Friday around 9:15 a.m. They said that tree fell into another one, and the momentum of that fall caused the first tree to swing back and strike Wyatt and pin him underneath it.

PASCAGOULA, MISS. — Mississippi Phosphates says it is restarting production at its Pascagoula fertilizer factory.The company shut down June 1 after an explosion killed Jeremy Moore of Moss Point and sent two other workers to the hospital. Another explosion in the same part of the plant killed a worker May 21.The company said Friday that an outside engineering firm that it had hired to investigate the deaths, Engineering Systems of Aurora, Ill., had inspected the plant before the restart. Mississippi Phosphates also said that it was cooperating with investigators from the U.S. Occupational Safety and Health Administration.
